General Surgery

    General surgery software development

A surgical case touches the clinic, the hospital and the billing team, and a missing consent or an outdated history can stop it at the door. Agnotic Technologies builds general surgery software that carries each case from booking to the end of its global period.

    General surgery software

    What is general surgery software?

General surgery software runs a surgical case from booking to the end of its global period: block time and case scheduling, surgeon preference cards, the pre-op record, operative notes and billing for the procedure and follow-up. Agnotic Technologies builds general surgery software for surgical practices and hospital departments, connected to the hospital EHR.

Most of the clinical record lives in the hospital EHR. The gaps sit between systems: the practice books a case the hospital has not scheduled, the consent is signed on paper, or a follow-up visit is billed when it is part of the global package.

This page covers the general surgery workflow. Ambulatory surgery center operations and operating room system comparisons are separate topics, and practice management across specialties is on our practice management software page.

    Pre-incision checklist

What must the record show before incision?

The checklist behind the pre-op screen. Hospital rules come from the Medicare conditions of participation for surgical services; site marking and the time-out follow the Joint Commission Universal Protocol as your hospital adopts it.

Before incisionRule that requires itField that captures it
Current history and physicalCompleted no more than 30 days before or 24 hours after admission or registration (42 CFR 482.51(b)(1)(i))H&P date and author pulled from the EHR, with the 30-day window shown on the case
Updated examinationWithin 24 hours after admission when the H&P was done in the prior 30 days (482.51(b)(1)(ii))H&P update note linked to the case, flagged when missing on the day of surgery
Informed consentA properly executed consent form for the operation in the chart before surgery, except in emergencies (482.51(b)(2))Signed e-consent with procedure, side, surgeon and witness, matched to the scheduled case
Site markingMarking of the procedure site under the hospital's Universal Protocol policyWho marked the site, when, and the side, matched to the consent and the schedule
Time-outA final check of patient, procedure and site before incision under the same policyTime-out record with each team member's confirmation and the time
Surgeon privilegesSurgical privileges delineated for every practitioner, with a roster kept (482.51(a)(4))Privilege list checked at booking, so a case cannot be scheduled outside it

Your hospital's policy may add items such as an anesthesia assessment or implant checks; we add them to the same screen during scoping.

    Case flow

How does a surgical case move from booking to the end of the global period?

One case, from the clinic decision to the last routine post-op visit.

    1. Decision for surgery

The surgeon records the decision in clinic, and the visit is coded so the decision visit bills correctly.

    2. Booking

The practice requests a slot in the surgeon's block or in open time, with procedure, side, expected duration and equipment.

    3. Pre-op

History and physical, labs, consent and any clearance letters are collected against the case, with gaps shown to the scheduler.

    4. Day of surgery

Site marking and the time-out are recorded, the preference card is shown to the team, and implants and supplies are scanned.

    5. Operative note

The surgeon completes a structured op note right after surgery and signs it.

    6. Global period

Post-op visits are scheduled and tracked as part of the package, and anything outside it is flagged for the right modifier.

What do we build into general surgery software?

Modules for the surgical case, each shaped by how surgeons, schedulers and billers hand work to each other.

    Case scheduling and block time

Block release rules, open time requests and case booking that checks surgeon privileges, room and equipment before a slot is requested.

    Surgeon preference cards

Instruments, supplies, positioning and setup per surgeon and procedure, versioned, with nurses able to propose changes after each case.

    Pre-op checklist

History and physical dates, consent, labs, clearances and site marking tracked on each case, with day-of-surgery gaps highlighted.

    Operative note templates

Structured fields for procedure, findings, specimens, implants, estimated blood loss and complications, plus free text where surgeons need it.

    Global period billing

Each case linked to its global period, so post-op visits inside it are not billed separately and separate services carry the right modifier.

    Post-op follow-up

Follow-up visits, wound checks and patient messages scheduled from the date of surgery, with missed visits flagged.

    Global period modifiers

Which modifiers separate work inside and outside the global period?

These CPT modifiers decide whether a visit or procedure around surgery is paid separately. The software asks for them at the moment the work is documented, not when the claim is rejected.

    01

    Modifier 57

Example: An evaluation on the day before or the day of a major operation that led to the decision to operate

Approach: The clinic visit is marked as the decision visit when the case is booked from it

    02

    Modifier 25

Example: A significant, separately identifiable evaluation on the same day as a minor procedure

Approach: The note must hold its own history and exam before the modifier can be added

    03

    Modifier 24

Example: An unrelated evaluation during the postoperative period

Approach: The visit needs a diagnosis unrelated to the surgery before it can bill outside the package

    04

    Modifier 58

Example: A staged or planned related procedure during the postoperative period

Approach: Linked to the original case as planned at the time of surgery, and a new global period starts

    05

    Modifier 78

Example: An unplanned return to the operating room for a related problem

Approach: Linked to the original case, and the original global period carries on

    06

    Modifier 79

Example: An unrelated procedure during the postoperative period

Approach: Needs an unrelated diagnosis and starts its own global period

    Failure points

Where do surgical cases get stuck?

Problems we design around on every general surgery build.

    Challenge

Cases are booked in the practice but not in the hospital's OR schedule.

    Agnotic approach

Booking requests go to the hospital through its scheduling interface or a tracked request queue, and the practice sees the confirmed time.

    Challenge

The history and physical is older than 30 days on the day of surgery.

    Agnotic approach

Each case shows the H&P date and the days left, and schedulers see cases at risk a week ahead.

    Challenge

The consent says left and the schedule says right.

    Agnotic approach

Procedure and side are entered once at booking and copied to the consent, the site marking record and the op note.

    Challenge

Preference cards are out of date.

    Agnotic approach

Cards are versioned, nurses propose changes after each case and the surgeon approves them.

    Challenge

Post-op visits are billed when they are part of the package.

    Agnotic approach

Visits inside the global period are flagged before charges are sent.

    Who it is for

Who do we build general surgery software for?

Surgical teams whose cases cross the clinic, the hospital and billing.

    General surgery practices

Groups operating at one or more hospitals who need booking, pre-op tracking and global period billing in one place.

    Hospital surgical departments

Department tools for block time, preference cards and op note templates that sit beside the hospital EHR.

    Multi-specialty groups and MSOs

Surgical service lines across locations, with shared scheduling and reporting.

    Health tech founders

Startups building surgical scheduling, consent or follow-up products who need HIPAA-ready engineering.

    Scope and cost

What drives the cost of general surgery software?

We do not publish prices. These are the things that change the size of a build, and we price them on the scoping call.

    Hospitals and EHRs

How many hospitals you operate at, and how each one exposes scheduling, documents and results.

    Scope of the case record

Booking only, or the pre-op checklist, op notes and global period billing as well.

    Consent and signatures

E-consent, witnesses, interpreters and how copies reach the hospital chart.

    Billing connection

Your practice management system or a clearinghouse connection we build.

    Patient messages

Pre-op instructions and post-op check-ins by text, email or app.

    Data migration

Preference cards, templates and open cases carried over from the current system.

    Engagement

How does a general surgery software engagement work?

We follow one real case end to end before we build.

    1.

    Frequently Asked Questions

No. The hospital EHR stays the record of the operation itself. We build the practice and department tools around it and connect them through the EHR's scheduling, document and FHIR interfaces where the hospital allows.
