IDN or health system
- Parent account for the whole system
- Executive contacts and strategic account plan
- System-wide contract coverage
- Roll-up of all child sites
Reps track surgeons in spreadsheets, hospital systems appear three times under different names, and the Open Payments report is rebuilt by hand every spring. Agnotic Technologies configures and extends Salesforce Sales Cloud for medical device and life sciences sales teams, built around the accounts, people and rules your market runs on.
Trusted by global innovators

A medtech Salesforce Sales Cloud setup keys healthcare professional and facility records to the NPI, models hospital systems, IDNs and GPO contracts as account hierarchies, aligns territories and quotas, and captures transfers of value for Open Payments. Agnotic Technologies builds this for device and life sciences sales teams, with patient data kept out.
Out of the box, Sales Cloud knows accounts, contacts and opportunities. It does not know that a surgeon operates at three hospitals, that those hospitals buy through one GPO contract, or that a lunch with the surgeon must be reported to CMS.
This page covers the commercial sales org. Patient and care management on Salesforce is covered on our Salesforce Health Cloud page.
Account hierarchy
Hospital buying is layered. The account model follows the layers, so every report can roll up or drill down.
Data model
Every report and integration depends on getting these objects and their keys right first. We agree the source of truth for each before any data is loaded.
| Object | Key identifier | Source of truth | What depends on it |
|---|---|---|---|
| HCP contact | NPI, checked against the NPPES record | NPPES for identity, your team for relationships | Open Payments reporting, call planning, territory assignment |
| Facility account | NPI for the organization, plus your ERP customer number | ERP for billing, CRM for relationships | Orders, quotes and account plans |
| IDN or health system parent | Your internal system ID | Your commercial operations team | Roll-up reporting, contract coverage, executive account plans |
| GPO contract | Contract number and tier | Contract management or ERP | Price on quotes, eligibility checks per facility |
| Opportunity | CRM ID linked to the facility and product line | CRM | Forecast, quota attainment, capital equipment pipeline |
| Transfer of value | CRM ID with the HCP's NPI and the event | CRM, reconciled with expense records | The annual Open Payments submission |
NPI and license data come from public registries, but the relationships between people, sites and contracts are yours, and they are where most clean-up time goes.
Transfer of value
The cheapest moment to get a record right is when the event happens. The flow we build follows that rule.
A meal, travel, sample or fee is logged in the mobile app against the attending HCPs, with the amount and date.
The system checks each attendee's NPPES record and credentials against the covered recipient types, so attendees who are not reportable are marked as such.
The rep chooses the nature of payment from the reportable categories, and the form follows from the event type.
Records are matched to expense reports so amounts agree with what finance paid.
Compliance reviews the year's records and exports them in the format the Open Payments submission needs.
PHI boundaries
A sales CRM is not built to hold patient data, and most companies do not want it there. These are the places it slips in.
Challenge
Case notes after a procedure
Agnotic approach
Structured fields for product and outcome, and free-text guidance that tells reps never to enter patient names or dates of birth.
Challenge
Field service and installation notes
Agnotic approach
Service notes are kept to device and site, and photo uploads are blocked on objects that do not need them.
Challenge
Complaints and adverse events heard by reps
Agnotic approach
A short intake that routes the report to your quality system instead of storing details in the CRM.
Challenge
Email and file attachments
Agnotic approach
Email-to-CRM rules and file types limited, with scans that flag likely patient identifiers for review.
Sales or service
Salesforce Sales Cloud is for teams that sell, working opportunities, quotes and forecasts, while Service Cloud is for teams that support customers, working cases, entitlements and knowledge articles. Agnotic Technologies sets up Sales Cloud for field and inside sales, Service Cloud for customer or technical support, and both on one org when a company runs the two teams.
| Team | Cloud | Core objects | Integration it needs |
|---|---|---|---|
| Field and inside sales | Sales Cloud | Accounts, contacts, opportunities, quotes, territories and forecasts | ERP for pricing, contracts and orders, with approved quotes sent back |
| Customer or technical support | Service Cloud | Cases, entitlements and service levels, knowledge articles and service channels | Product, installed base and service records, and for medtech the complaint hand-off to the quality system |
| Both teams | Both clouds on one org | Shared accounts and contacts, with opportunities for sales and cases for support | One account model and one integration layer, so sales sees open cases and support sees the installed base |
For a device maker, a complaint a support agent logs in Service Cloud may need to reach the quality system for complaint handling, so we route it there rather than closing it as an ordinary case.
Configuration first, code where the configuration runs out. Each item can be scoped on its own.
Territories by geography, account or product line, with quota roll-ups that match how reps are paid.
Records matched to NPPES, deduplicated and loaded from spreadsheets or a previous CRM.
Meals, travel, samples and fees logged against the HCP's NPI at the time of the event.
Pricing, contracts and order history from your ERP, and approved quotes sent back.
Validation and field design that keep patient details out of a sales system.
Automated checks on custom code and flows before each Salesforce release reaches production.

Scope drivers
We size the work on a scoping call. Licences are bought from Salesforce and are not part of our estimate.
Spreadsheets and a previous CRM with duplicates take longer to match than a clean export.
A few direct hospital accounts are simpler than IDNs with many sites and several GPO contracts.
Read-only pricing is smaller than two-way quote and order sync.
Your compliance team's reporting policy sets how much validation the capture flow needs.
Each line can bring its own territories, quotas and opportunity stages.
Short facts the data model is built around, each linked to its primary source.
Applicable manufacturers must report to CMS, every year, the direct and indirect payments and other transfers of value they made to covered recipients during the preceding calendar year.
Source: 42 CFR 403.904(a) (eCFR)Covered recipients for Open Payments are physicians, physician assistants, nurse practitioners, clinical nurse specialists, certified registered nurse anesthetists and certified nurse-midwives who are not the manufacturer's employees, and teaching hospitals.
Source: 42 CFR 403.902 (eCFR)Each Open Payments record must include the covered recipient's name and primary business address, and for individual recipients their specialty, National Provider Identifier as listed in NPPES and a state license number, along with the amount, date, form and nature of the payment.
Source: 42 CFR 403.904(c) (eCFR)The National Provider Identifier is a 10-position numeric identifier with a check digit in the 10th position and no intelligence about the provider in the number.
