Challenge
Bulk $export volume overwhelms naive pipelines
Agnotic approach
We build the async $export flow with paging, backpressure, and idempotent ingestion so large datasets land reliably.
We integrate 1upHealth to aggregate patient data across thousands of payers and health systems — its FHIR R4 APIs, patient-authorized connections, and bulk FHIR $export — into your platform.
Trusted by global innovators

1upHealth is a FHIR-native patient data platform. It lets patients authorize connections to their payers and providers, aggregates the returned records into a normalized FHIR R4 store, and exposes them through standard FHIR APIs — including the bulk FHIR $export used for population-scale data movement and the CMS Patient Access and Provider Directory API patterns payers must support.
The engineering challenge is data at two scales: the individual patient-authorized connection flow, and population-level bulk export that can return enormous FHIR datasets. We build both — the OAuth-based patient connection experience and the asynchronous bulk $export pipeline — plus the normalization and storage that turn aggregated FHIR into something your analytics and product can actually use.
See how we design patient-authorized aggregation and population-scale bulk $export on top of 1upHealth.

Challenge
Patient connection drop-off during OAuth
Agnotic approach
A clear, well-designed connection experience that maximizes successful authorizations and re-connections.
Challenge
Aggregated data is inconsistent across sources
Agnotic approach
A normalization and reconciliation layer over the aggregated FHIR so your product sees consistent resources.
Challenge
CMS interoperability rules are misread
Agnotic approach
We build to the Patient Access and Provider Directory API patterns explicitly, with the required audit trails.
The exact FHIR surface behind patient data aggregation at individual and population scale.
Normalized FHIR R4 access to aggregated patient records — Patient, Coverage, Claim, Observation, Condition, and more.
Patient-authorized OAuth connections to thousands of payers and provider systems for data aggregation.
Asynchronous bulk $export for population-level data movement into analytics, warehouses, and data lakes.
CMS Patient Access and Provider Directory API patterns for payer interoperability compliance.

Aggregate cross-provider history for care-management teams.
Build normalized FHIR datasets for model training and research.
1upHealth spans patient-authorized connections and population bulk export. We build for both scales deliberately.

Standards we build against
Patient-connection UX and population-scale bulk export treated as distinct engineering tracks.
We define whether you need patient-authorized aggregation, population bulk export, or both, and which FHIR resources matter.
We build the OAuth patient connection flow and the FHIR API integration into your data store.
We validate aggregation and bulk $export against sandbox data, including large-dataset performance.
Production rollout with connection-health tracking, export job monitoring, and data-freshness checks.
Lera Health is related proof of compliant, integration-heavy delivery — a privacy-first, data-driven platform built end to end. 1upHealth work draws on the same strengths: FHIR data modelling, patient-consented aggregation, and HIPAA-grade handling of PHI.

Real aggregation at both scales, not just an API demo.
Patient consent, CMS interoperability patterns, and audit trails built in from the start.
We work fluently in FHIR R4, bulk $export, and the CMS Patient Access API requirements.
We engineer the asynchronous bulk pipeline so population-level export does not break under volume.
We turn aggregated FHIR into usable product features and analytics, not just a raw data dump.
Tell us whether you need patient connections, bulk export, or both. We will return a realistic aggregation plan.
